Requirements for Temporary Food Facilities in the County of Santa Clara

Forms that food vendors must submit prior the contest:

Application to operate a Temporary Food Facility (TFF)

Teams who wish to vend will need to fill out the left hand side of page 1, sign (and date) page 1 (we've pre-filled the right hand side)
and fill out the top half of page 2 with ANY food items you MAY serve the public at this event, listing more and not serving them is insignificant compared to the reverse.

Then they can be scanned (if you have that option available) and emailed to

or faxed to 408-779-5405

by April 15 2011

This is the only form we need in advance.
