Peoples Choice (PC) Forms - Required for PC participation
If you would like to participate in the peoples choice (selling samples to the public) you will need the
following forms.
We may have some of the on-site deliverable forms available on-site the day of the contest - but we might
not. So print out your copy, fill it out, and bring it with you.

Forms that must be submitted prior the
contest:
Application to operate a Temporary Food
Facility (TFF)
Teams who wish to vend will need
to fill out the left hand side of page 1, sign (and date) page 1 (we've pre-filled the right hand side)
and fill out the top half of page 2 with ANY food items you MAY serve the public at this event, listing
more and not serving them is insignificant compared to the reverse.
Then they can be scanned (if you have that option available) and emailed to

or faxed to 408-779-5405
by April 15 2010
